Requirements

  • Active RN license (New Jersey)
  • ADN or BSN
  • 2+ years of nursing experience (IDD, home health, community health, or case management preferred)
  • Strong clinical assessment skills
  • Excellent communication and collaboration abilities
  • Valid driver’s license & reliable transportation

Responsibilities

  • Manage a dedicated caseload across Arc Mercer residential homes, tailoring visit frequency to individual acuity.
  • Perform in-person nursing assessments, observe behavioral/physical changes, and escalate care early.
  • Strategize with staff before appointments, prep documentation, and ensure closed-loop follow-through.
  • Lead proactive monitoring for diabetes, hypertension, respiratory conditions, seizure disorders, and more.
  • Own medication reconciliation, MAR accuracy, interaction monitoring, and medication error investigations.
  • Coach DSPs and Residential Managers through real-time, on-site clinical education.
  • Partner with Behavioral Services to identify medical causes behind behavioral changes.
  • Participate in discharge planning, secure equipment, and ensure seamless home transitions.
  • Join weekly interdisciplinary huddles to review trends, risks, and upcoming procedures.
  • Track key health metrics, appointment completion, medication errors, and readmission trends.
