About The Position

The PRN Outpatient Behavioral Health Therapist will focus on Care, Community, and Compliance. This involves collaborating with an interdisciplinary team (psychiatric physician, RN Director, CNA) to deliver high-quality care to older adult patients in a geriatric intensive outpatient setting, providing therapeutic services through group, individual, and family therapy. The role also includes participating in community education on behavioral health for older adults and working with the program team to facilitate treatment team meetings, develop patient-centered treatment plans, and ensure compliant documentation within prescribed timelines.

Requirements

• License in Social Work or Counseling as recognized by the state; may include LISW, LCSW, LPC, LMFT, etc. – required

Responsibilities

• Collaborate with a small, interdisciplinary team including a psychiatric physician, an RN Director, and a CNA to provide high-quality care to Older Adult patients as ordered.
• Provide therapeutic services utilizing group, individual, and family therapy sessions in a geriatric intensive outpatient environment.
• Participate in community education for local medical facilities and other community groups on the importance of behavioral health for all, including older adults.
• Work with the program team to facilitate treatment team meetings.
• Develop and update the patient-centered treatment plan.
• Produce compliant documentation in a prescribed timeline.
• Work with kindness and accountability to enhance the quality of life for our patients.
