About The Position

Provide therapeutic interventions and case management services to youth who are mentally or emotionally disturbed and their families. Interventions include individual, group, and family therapy as well as goal setting and monitoring and de-escalation techniques. Case management includes compliance with documentation requirements including monitoring of files. Serve as liaison for clients and their families with internal and external resource agencies.

Requirements

  • Master’s in social work, counseling or related field.
  • Must be licensed as an LCSW, LPC or related, as approved under Virginia Board of Counseling, or under supervision toward licensure.
  • Two to three years direct experience providing direct clinical experience, behavioral interventions, and case management.

Responsibilities

  • Provide direct clinical services to CFHC youth to include a minimum of three individual therapy sessions per workweek (Saturday through Sunday) and up to four group therapy sessions.
  • Conduct family therapy sessions once per week, if required. Sessions should be completed in person, whenever possible, or conducted via telehealth. Family therapy must be in person or via telehealth, no less than once per four-week period; other sessions may be conducted via telephone. Session times may require nights or weekends.
  • Provide guidance and direction to suite staff in delivering quality services to youth and families consistent with practice principles and strengths-based philosophy, licensing standards, and professional ethics.
  • Provide daily treatment guidance and direction by monitoring client’s activities in a manner that develops and nurtures personal values and an effective system of communication. Present daily treatment interventions and options to promote social, school, and discharge success to a lesser level of care.
  • Represent the agency and client at required FAPT, court hearings and other necessary meetings by displaying an appropriate professional appearance and advocating appropriately for both the client and the agency.
  • Actively support the team as a member of Suite Leadership through weekly team meetings, therapeutic mentoring, coaching and guidance on individual treatment issues as well as providing feedback to the team on group issues.
  • Responsible for providing case management services by ensuring compliance with agency, licensing, accreditation, and Medicaid policies and procedures by completing all necessary clinical documentation within established deadlines. Ensure required documentation is clinically relevant, complete, accurate and within agency, licensing, accreditation, and Medicaid policies and procedures.
  • Establish and maintain positive professional relationships with legal guardians, social workers, probation officers, families and referral sources to advocate and support our clients.
  • Ensure appropriate attention to crisis management issues by promptly responding to emergencies and potentially unsafe situations with CFHC youth by providing therapeutic interventions for clients in crisis, to include the use of approved verbal and physical intervention techniques, and Agency behavior management practices while on-campus during business hours or on-call, if required.
  • Interprets psychiatric treatment and compassionately communicates it to youth's family to help to reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment.
  • Provide suggestions for treatment interventions and options including behavior support, social skill development, problem-solving, anger management, community responsibility, increased impulse control, appropriate peer relations, and crisis intervention while upholding professional standards.
  • Assist Clinical Case Manager in the development, revision, and implementation of Individualized Treatment Plans, which identify individual needs, strengths, goals and objectives and that best meet the needs of the client.
  • In conjunction with the Treatment Team, determines weekly progress and discuss treatment direction for each youth.
  • Maintain on-going therapeutic relationship with each youth in order to assist them in their process of meeting their treatment goals.
  • Document all clinical and group session progress notes as required by agency, licensing, accreditation, and Medicaid policies and procedures.
  • Provide discharge planning including determining target discharge date.
  • Responsible for the upkeep, accuracy, and completeness of all client files. Maintain ongoing documentation of all collateral contacts.
