About The Position

Trinity Hospice Dallas is seeking a PRN RN to support patients in the South Dallas - Lancaster, Desoto, Mansfield areas. The ideal candidate will have RN experience working for a hospice agency/program. Trinity Hospice has a strong promote-from-within culture, significant census growth, and approachable leaders. This role focuses on patient comfort, dignity, and quality of life, involving patient assessments, symptom management, care plan adjustments, family support and education, and efficient EMR documentation. The nurse will advocate for patients while maintaining autonomy in structuring their day.

Requirements

RN experience working for a hospice agency/program is required
Active RN license in the state you’ll practice
Graduate of an accredited nursing program (ADN or BSN)
2+ years of RN experience required
Strong clinical judgment, compassion, and communication skills
Reliable transportation, valid driver’s license, auto insurance, and CPR certification
Must live in the South Dallas - Lancaster, Desoto, Mansfield areas

Responsibilities

Serve as the primary nurse coordinating care for assigned patients
Develop, implement, and update individualized plans of care
Determine visit frequency based on patient acuity and family needs
Provide guidance to Hospice Aides and LPNs to ensure excellent care
Communicate changes in condition to physicians and the care team
Participate in on-call rotation
Assess patient needs, manage symptoms, and adjust plans of care as conditions change
Coordinate closely with physicians, hospice aides, LPNs, social workers, and chaplains
Support and educate families
Document care clearly and efficiently in our EMR, Consolo
Advocate for patients while maintaining autonomy in how you structure your day
