About The Position
Trinity Hospice Dallas is seeking a PRN RN to join their top-notch team. In 2025, they successfully completed a deficiency-free full book survey. The team has a strong promote-from-within culture, an office with significant census growth, and approachable, empathic leaders. This role will support patients in the South Dallas - Lancaster, Desoto, Mansfield areas, and candidates must reside in these areas. A day in the life involves visiting patients in their homes, focusing on comfort, dignity, and quality of life, assessing patient needs, managing symptoms, adjusting plans of care, coordinating with the interdisciplinary team, and educating families. The role requires clear and efficient documentation in the EMR (Consolo) and advocating for patients while maintaining autonomy in daily scheduling.
Requirements
- Active RN license in the state you’ll practice.
- Graduate of an accredited nursing program (ADN or BSN).
- 2+ years of RN experience required.
- RN experience working for a hospice agency/program is required.
- Strong clinical judgment, compassion, and communication skills.
- Reliable transportation, valid driver’s license, auto insurance, and CPR certification.
Responsibilities
- Serve as the primary nurse coordinating care for assigned patients.
- Develop, implement, and update individualized plans of care.
- Determine visit frequency based on patient acuity and family needs.
- Provide guidance to Hospice Aides and LPNs to ensure excellent care.
- Communicate changes in condition to physicians and the care team.
- Participate in on-call rotation.
