About The Position

Pediatric Nursing Specialists of Indiana (PNS), a Home Care Network (HCN) agency, is seeking a PRN RN Case Manager for a leadership position with long-term employment potential and opportunities for growth. This role involves providing RN Case Manager services, care, and travel to clients throughout Indiana. The position reports to the Indiana Regional Program Administrator and the Director of Clinical & Administrative Operations. There is a defined path for advancement to various leadership roles within the agency. The work includes case management, starts of care (SOC), admissions, recertifications, transitional care, resumptions of care (ROC), clinical reviews, assessments, new hire orientations, in-service and supervisory reviews, and contributing to training, education, and care practices. The agency is nurse-owned and locally led, prioritizing patient care and wellbeing. This is a PRN position with long-term employment opportunities, offering flexible and customizable schedules with no overnight shifts or overnight travel required. Mileage reimbursement is provided.

Requirements

  • At least 1 year of experience in Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience at/with a home health agency or in-home care provider (or equivalent).
  • At least 1 year of Case Management experience and home health agency (or equivalent) experience are required.
  • Must have experience with OASIS and NON-OASIS assessments.
  • Graduate of an accredited School of Nursing.
  • Minimum of one (1) year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Indiana.
  • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience.
  • 1+ years of paid home health / in-home care experience is also required.
  • Minimum of one (1) year Experience in case management is required.
  • CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties).
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
  • Willing to consent to a background check, including a criminal record check, and an employment and education verification.

  • 2+ years of both home health/acute care experience and paid home health/in-home care experience are preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.
  • Experience and willingness to serve clients of all ages (infants, pediatrics, adults, seniors).
  • Home care experience preferred.

Responsibilities

  • Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document/Communicate/Coordinate patient status/change in condition to the care team.
  • Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Provide RNCM services, care, and travel as needed to all clients throughout Indiana.
  • Conduct case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services.
  • Conduct new hire orientations, in-service and supervisory reviews.
  • Provide input to influence training and education, care practices and outcomes, and the success of clients, employees, and the business.
  • Provide occasional Skilled Nursing visits.
  • Coordinate and provide patient care and ensure optimal outcomes.
  • Oversee patient services and provide expert nursing care.
  • Ensure successful communication with and between physicians, agency staff, service administrators, other caregivers and clinicians, and the client and their family.
