Job Description: Home Health Registered Nurse (RN) Case Manager Newburyport Territory/Full Time (Monday through Friday 8a-5p) At Elara Caring, we believe the best care happens where patients feel most comfortable—at home. As we continue to grow in the Newburyport market, we are expanding our clinical team and seeking a dedicated Registered Nurse (RN) – Home Health who is ready to be part of a mission-driven organization that values clinical excellence, autonomy, and meaningful patient relationships.

Requirements

  • Current, unrestricted Massachusetts Registered Nurse (RN) license
  • Minimum of one (1) year of nursing experience in a clinical care setting
  • Ability to commit to at least one full day (8 hours) per week
  • Valid driver's license, reliable transportation, and current auto insurance
  • Willingness to travel throughout the assigned territory

Responsibilities

  • Completing skilled nursing assessments and developing individualized plans of care
  • Performing admissions, routine visits, recertifications, and discharge visits
  • Evaluating patients' physical, psychosocial, environmental, and educational needs
  • Coordinating care with physicians, caregivers, and interdisciplinary team members
  • Monitoring patient progress and identifying changes in condition
  • Updating and revising care plans to ensure optimal outcomes
  • Educating patients and families on disease management, medications, and self-care techniques
  • Maintaining accurate and timely clinical documentation
  • Helping prevent avoidable hospitalizations through proactive intervention and patient education
  • Delivering compassionate, patient-centered care while promoting safety and independence
