PRN Registered Nurse
About The Position
Overture Home Care is seeking a compassionate and dedicated Registered Nurse for a PRN (as-needed) position in the Dallas area. The role involves assisting with client assessments for seniors with diverse needs, specializing in Alzheimer's/Dementia care, hospice, and general geriatric care. As a vital member of the healthcare team, the RN will play a crucial role in providing top-notch care to seniors throughout the Dallas area. Requirements
- Must have a full, active license as a Registered Nurse.
- Minimum of two years related work experience in a geriatric healthcare setting required.
- Flexibility as needed when it comes to assessments and scheduling trainings
- Demonstrates effective verbal and written communication skills
- Ability to work independently
- Demonstrates initiative, problem solving ability, adaptability and flexibility
- Clinical skills: documentation of care, medication administration, infection control precautions, assessing patient educational needs, physical assessment, obtaining vital signs, care coordination.
Responsibilities
- Conduct in-home and/or facility-based client assessments as needed to determine care needs and eligibility
- Collaborate with intake, case management, and nursing teams to ensure seamless care coordination
- Evaluate client conditions and recommend appropriate levels of care and services
- Maintain accurate and timely documentation of all assessments and clinical findings
- Communicate effectively with clients, families, and referral sources to support care planning
- Ensure compliance with all state regulations and company policies during assessments
- Support care transitions from hospitals, rehab facilities, or other care settings
- Build and maintain professional relationships with referral partners and healthcare providers
- Provide clinical insight and support nursing and caregiving staff as needed
- Participate in case reviews and contribute to ongoing quality improvement initiatives
- Travel within the Dallas area to complete assessments as required
