About The Position

Overture Home Care is seeking a compassionate and dedicated Registered Nurse for a PRN (as-needed) position in the Dallas area. The role involves assisting with client assessments for seniors with diverse needs, specializing in Alzheimer's/Dementia care, hospice, and general geriatric care. As a vital member of the healthcare team, the RN will play a crucial role in providing top-notch care to seniors throughout the Dallas area. Requirements

  • Must have a full, active license as a Registered Nurse.
  • Minimum of two years related work experience in a geriatric healthcare setting required.
  • Flexibility as needed when it comes to assessments and scheduling trainings
  • Demonstrates effective verbal and written communication skills
  • Ability to work independently
  • Demonstrates initiative, problem solving ability, adaptability and flexibility
  • Clinical skills: documentation of care, medication administration, infection control precautions, assessing patient educational needs, physical assessment, obtaining vital signs, care coordination.

Responsibilities

  • Conduct in-home and/or facility-based client assessments as needed to determine care needs and eligibility
  • Collaborate with intake, case management, and nursing teams to ensure seamless care coordination
  • Evaluate client conditions and recommend appropriate levels of care and services
  • Maintain accurate and timely documentation of all assessments and clinical findings
  • Communicate effectively with clients, families, and referral sources to support care planning
  • Ensure compliance with all state regulations and company policies during assessments
  • Support care transitions from hospitals, rehab facilities, or other care settings
  • Build and maintain professional relationships with referral partners and healthcare providers
  • Provide clinical insight and support nursing and caregiving staff as needed
  • Participate in case reviews and contribute to ongoing quality improvement initiatives
  • Travel within the Dallas area to complete assessments as required
