About The Position
Commonwise is a care management and home care company based in Charlottesville, VA, with offices in Richmond, Williamsburg, and Charleston, SC. The company provides professional geriatric care management to older adults through registered nurses and delivers premium home care services through nurse-aides. This role is for a PRN (as needed) Registered Nurse who will serve as a Care Manager. The ideal candidate is a Registered Nurse licensed in Virginia, dedicated to providing the best care, open to a non-clinical work environment, a strong communicator, creative problem solver, and compassionate. Requirements
- Registered Nurse licensed in Virginia
- At least 5 years of experience in the field of nursing
- Excellent written and verbal communication skills

Responsibilities
- Provide senior housing & relocation assistance to clients
- Provide supportive consulting to clients
- Manage family relationships for clients
- Manage healthcare crises for clients
- Coordinate healthcare for clients
- Conduct home safety evaluations for clients
- Provide life care management to clients
- Serve as account managers for clients receiving home care services
- Serve as account managers for nurse-aides providing direct care
