About The Position
Commonwise is a care management and home care company based in Charlottesville, VA, with additional offices in Richmond, Williamsburg, and Charleston, SC. The company provides professional geriatric care management to older adults through registered nurses and delivers premium home care services through industry-leading nurse-aides. The Care Managers in this role provide a range of services including senior housing & relocation assistance, supportive consulting, family relationship management, healthcare crisis management, coordination of healthcare, home safety evaluations, and life care management to clients. They also act as account managers for clients receiving home care services and for the nurse-aides providing direct care. Requirements
- Registered Nurse licensed in Virginia
- At least 5 years of experience in the field of nursing
- Excellent written and verbal communication skills
- Dedicated to providing clients the best care
- Looking for or open to a non-clinical work environment
- Strong communicator and creative problem solver
- Compassionate
- Knowledgeable of the healthcare landscape

Responsibilities
