About The Position
PRN Registered Nurse (RN) – Homecare Hospice of South Texas (HOST) Victoria, Texas and surrounding service areas At Hospice of South Texas, we are dedicated to providing compassionate end-of-life care with dignity, respect, and excellence. Requirements
- Current Texas RN license
- Current Texas Driver's License
- Reliable transportation
- Current auto liability insurance
- Ability to travel locally throughout service area
- CPR certification
- Ability to lift up to 50 lbs. and assist with patient mobility
- Ability to work flexible schedule and occasional on-call needs
Nice To Haves
- Minimum two years RN experience in a clinical setting preferred
- Hospice experience required
Responsibilities
- Perform comprehensive patient assessments to identify physical, emotional, psychosocial, and environmental needs
- Develop, implement, and update individualized plans of care
- Provide direct nursing care and symptom management
- Coordinate services with physicians, aides, LVNs, social workers, chaplains, and other team members
- Educate patients and families regarding care needs and disease progression
- Ensure accurate and timely clinical documentation
- Maintain compliance with hospice regulations and payer requirements
- Participate in on-call rotation as assigned
- Promote HOST values, standards, and quality patient care
- Other duties as assigned
