About The Position

The Patient Care Coordinator is a key administrative and front-office partner who creates a welcoming, organized, and supportive experience for patients and families from the moment they enter our clinics. Serving as the first point of contact, this role plays a critical part in ensuring smooth clinic operations, clear communication, and a positive patient experience. The Patient Care Coordinator works closely with clinicians and care teams to support scheduling, registration, documentation, and day-to-day clinic flow. This role is ideal for someone who is detail-oriented, service-driven, and energized by supporting both people and processes in a fast-paced healthcare environment.

Requirements

  • High school diploma or GED required; additional education or equivalent experience is a plus
  • 1+ year of experience in an administrative, front desk, customer service, or patient-facing role
  • Comfort using computer systems and scheduling platforms (e.g., Microsoft Office, EMR or similar systems)
  • Strong organizational skills with the ability to manage multiple priorities and maintain attention to detail
  • Clear, professional, and empathetic communication skills — both written and verbal
  • Ability to remain calm, flexible, and collaborative in a fast-paced environment
  • A proactive mindset with a commitment to accountability, teamwork, and continuous improvement

Responsibilities

  • Serve as the primary front desk point of contact, creating a welcoming and professional experience for patients and families
  • Manage check-in, registration, and scheduling workflows, ensuring accuracy and efficiency
  • Collect, verify, and maintain patient demographic and insurance information in the EMR
  • Handle administrative tasks including phone calls, documentation, scheduling requests, and visit preparation
  • Collect patient payments, process transactions accurately, and answer basic billing and payment-related questions.
  • Communicate and collaborate with clinicians and clinic staff to support smooth clinic operations
  • Support patient outreach and follow-up related to scheduling and care coordination needs
  • Maintain a clean, organized front-office environment and assist with opening and closing routines
  • Provide cross-coverage support, assist with training, and contribute to process improvements as needed
  • For PCCs with the clinical qualifications, there is the opportunity to flex up and support the back office as needed
  • Maintain a minimum availability of three shifts per month.
  • Work at least one weekend shift per month.
  • Work at least one enterprise-designated winter holiday and one designated high-impact day during the applicable period.
  • Maintain a high level of engagement with the organization between scheduled shifts.
  • Regularly review company emails, communications, and operational updates to remain informed and connected to the team.
