About The Position

Provide palliative care consultation and treatment to patients with serious and life-limiting illnesses in their homes, skilled nursing facilities, assisted living facilities, outpatient clinic, and hospital. This role involves providing palliative care consultation and follow-up, including education, therapeutic communication, and management of symptoms related to life-limiting conditions. Care is performed in coordination with an interdisciplinary team, which includes a collaborating physician, registered nurse, and social worker, with support from a chaplain and musical therapist. The provider will also participate in ongoing learning, coordinate care with referral sources, maintain professional appearance, and work towards performance goals. Accurate patient records must be created and maintained within the electronic medical record. The provider will serve as a clinical resource for staff and community providers, maintain current clinical education and licensure, and perform other duties as requested.

Requirements

  • Masters or Doctorate Level education.
  • Current NP or PA Board Approved Certification.
  • Current NC State License in good standing.
  • Current DEA Registration in good standing.
  • Meet and maintain eligibility requirements to be appointed medical staff and qualify for privileges at partner facilities.
  • Valid driver’s license and proof of automobile insurance.
  • Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation.
  • Knowledge of HIPAA privacy rules and regulations.
  • Knowledge of electronic medical records systems.
  • Basic data entry skills.

Nice To Haves

  • 2 years of clinical experience as a medical provider is preferred.
  • Palliative care experience is preferred.

Responsibilities

  • Provide palliative care consultation and follow-up to patients and their involved persons, including education, therapeutic communication, and management of symptoms related to life-limiting conditions.
  • Coordinate care with an interdisciplinary team, including a collaborating physician, registered nurse, and social worker, with support from a chaplain and musical therapist.
  • Provide Palliative Care in patient’s homes, ALF/SNF, outpatient clinic, and hospital locations and submit mileage reimbursement monthly.
  • Participate in ongoing learning, including symptom management of complex illnesses, communication skills, and prognostication.
  • Coordinate care and communicate with referral sources.
  • Maintain professional appearance and presentation.
  • Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies.
  • Actively participate in Interdisciplinary Team meetings for development and evaluation of the clients plan of care.
  • Create and maintain accurate patient records within the electronic medical record and complete per policy timeframe.
  • Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation.
  • Serve as clinical resource for staff of Four Seasons, staff at facilities served, and providers in the community.
  • Maintain current clinical education and appropriate licensure.
  • Perform other duties as requested or assigned.
  • Practice safety and infection control methods.
  • Maintain documentation of collaborative practice / supervision as appropriate for clinical licensure.
  • Occasional night and weekend phone call coverage, shared among a large clinical team of providers.
  • Willingness to drive a variety of terrain and miles between patient locations.
