Provide palliative care consultation and treatment to patients with serious and life-limiting illnesses in their homes, skilled nursing facilities, assisted living facilities, outpatient clinic, and hospital. This role involves providing palliative care consultation and follow-up, including education, therapeutic communication, and management of symptoms related to life-limiting conditions. Care is performed in coordination with an interdisciplinary team, which may include a collaborating physician, registered nurse, and social worker, with support from a chaplain and musical therapist. The provider will also submit mileage reimbursement monthly, participate in ongoing learning, coordinate care with referral sources, maintain professional appearance, and work towards performance goals and company values. Accurate patient record maintenance in the electronic medical record is essential, as is practicing within the scope of license and serving as a clinical resource. Maintaining current clinical education and licensure is also required.
