Requirements

  • Current Occupational Therapist (OT) license in the state of practice
  • Strong assessment, communication, and documentation skills
  • Ability to work independently and manage a flexible schedule
  • Reliable transportation and valid driver's license

Nice To Haves

  • Home health experience preferred but not required
  • Current CPR certification preferred

Responsibilities

  • Evaluate patients' functional abilities and develop individualized treatment plans
  • Provide therapeutic interventions to improve daily living skills and overall independence
  • Educate patients and caregivers on adaptive techniques, safety strategies, and equipment use
  • Recommend home modifications to enhance safety and functionality
  • Monitor patient progress and modify treatment plans as needed
  • Maintain accurate and timely clinical documentation in accordance with company and regulatory standards
  • Collaborate with physicians, caregivers, and interdisciplinary team members to ensure quality care
