Requirements

Current Occupational Therapist (OT) license in the state of practice
Strong assessment, communication, and documentation skills
Ability to work independently and manage a flexible schedule
Reliable transportation and valid driver's license

Nice To Haves

Home health experience preferred but not required
Current CPR certification preferred

Responsibilities

Evaluate patients' functional abilities and develop individualized treatment plans
Provide therapeutic interventions to improve daily living skills and overall independence
Educate patients and caregivers on adaptive techniques, safety strategies, and equipment use
Recommend home modifications to enhance safety and functionality
Monitor patient progress and modify treatment plans as needed
Maintain accurate and timely clinical documentation in accordance with company and regulatory standards
Collaborate with physicians, caregivers, and interdisciplinary team members to ensure quality care
