This position is responsible for providing crisis intervention and case management services to individuals in need of behavioral health and/or co-occurring services. The role involves responding to crisis calls, de-escalating situations, linking clients to appropriate care, and providing advocacy to prevent hospitalization. The case manager will establish rapport with clients, assess needs, deliver face-to-face services, and assist clients in accessing community resources for independent functioning and safety planning. This role also involves working with inpatient facilities for discharge planning and follow-up care, maintaining strict confidentiality, and documenting services according to agency policy. The position may require evening or weekend hours and providing client transportation.
