About The Position

Concerto Renal Services is an industry leader in nursing home dialysis, rapidly expanding with a mission to reimagine dialysis care, one community at a time. Their model focuses on ensuring those with end-stage renal disease receive the best possible care through a more convenient, centralized, continuous care model. Concerto is seeking a PRN Licensed Practical Nurse to join its team. The PRN Licensed Practical Nurse functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients. This role works under the supervision of a Registered nurse in accordance with Concerto’s policies, procedures, and training and in compliance with regulations set for by state and federal agencies.

Requirements

Licensed Practical Nurse required
Current CPR Certification or completion upon starting in unit.
Able to meet physical demands of the position.
Minimum of (1) years of Licensed Practical Nurse experience in dialysis.

Responsibilities

Provides hands-on care to assigned patients under the supervision of a licensed nurse, including initiating and discontinuing dialysis treatments per prescribed orders.
Obtain pre and post treatment vitals, weight and vascular access assessment. Report any unusual changes or findings to the Registered nurse.
Sets up, operates, and performs safety checks on hemodialysis machines; administers heparin as permitted by state law. Initiate and discontinue dialysis treatment according to prescribed orders and policy.
Accurately documents patient treatment details including vital signs, treatment parameters, and change in condition, and prepares laboratory specimens as prescribed.
Delivers excellent service to patients, families, and staff, while maintaining a clean and safe work environment.
