About The Position

The Licensed Practical Nurse (LPN) is responsible for providing nursing care services and case management to clients and families by implementing nursing interventions set forth in the plan of care, according to prescribed policies and procedures of the organization, including all applicable federal, state, and accreditation regulations. This role reports to the Clinical Leader.

Requirements

  • Current LPN license in good standing in the state(s) in which practicing.
  • 12 months facility/agency experience in nursing as an LPN preferred.
  • Continuous travel, necessary on a daily basis.
  • Valid driver’s license.
  • Automobile insurance.
  • Qualified to drive under the organization’s motor vehicle check.
  • Good verbal and written communication skills.
  • Good comprehension skills.
  • Good computer skills.
  • Good interpersonal skills.
  • Ability to speak, read, write and understand English.

Responsibilities

  • Provides direct patient care as defined in the State Nurse Practice Act and as ordered by the physician.
  • Implements the plan of care initiated by the registered nurse.
  • Provides accurate and timely documentation consistent with the plan of care.
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participates in the coordination of home health services, appropriately reporting identified needs for other disciplines to the registered nurse, RN Case Manager, and/or Director of Clinical Management.
  • Provides mentoring, coaching, and education to direct care staff, under the supervision of the RN.
  • Uses equipment and supplies effectively and efficiently.
  • Participates in personal and professional growth and development.
  • Participates in on-call rotation.
  • Participates in interdisciplinary team meetings to coordinate the care of the client and family; exchange information and problem solve; and receive staff support and education.
  • Demonstrates and maintains a positive, professional manner, manages time efficiently and effectively, provides leadership as a role model to promote a team approach, collaborates and participates in policy creation and appropriate problem-solving methods.
  • Adheres to all policies, procedures, terms, and conditions set forth in the National Church Residences’ Employee Information Guide (EIG), the Code of Conduct, as well as any facility handbook, including but not limited to, corporate compliance, drug-free workplace, safe work practices, and all federal, state, local regulations and laws.
  • Ensures all care is provided with respect and dignity for the residents/clients, their families, and the staff, with respect to Resident Rights.
  • Reports all complaints made by residents/clients and/or families to the appropriate supervisors.
  • Reports all allegations of abuse, misappropriation of funds/resident property, and/or any other corporate compliance, drug-free workplace, safe work practices, all federal, state, and local regulations and laws.
  • Completes all annual education requirements timely, including but not limited to Compliance, Privacy, Security, and HIPAA training.
  • Exhibits behavior as set forth by the Code of Conduct in the performance of their duties.
  • Performs other duties as assigned.
