About The Position
KORT in New Albany, IN is seeking a PRN Home Health nurse case manager. KORT is Kentucky/Indiana's premier provider of outpatient physical and occupational therapy specializing in orthopedics, sports and industrial medicine. Since 1987, KORT has provided clinical and customer service excellence to patients in our clinics, at employer sites, and with high school, college, and professional sports. In addition, KORT Home Health also contracts their home health services in Kentucky including: post-operative physical therapy; fall prevention; vestibular rehabilitation; wound care; and more.
Requirements
- Be a registered nurse (R.N.) with a current Indiana license.
- Minimum of one year of nursing experience as a R.N.
- Work positively and favorably with patients, families, and staff.
- Strong verbal and written communications skills
- Flexible with diverse client assignments.
- Current automobile liability insurance and current unrestricted drivers license.
- CPR certification is mandatory immediately after employment
Nice To Haves
- One (1) year of Home Health experience preferred.
- OASIS E experience/proficient
