About The Position

If you're a home health RN who wants steady, predictable hours without giving up meaningful, hands-on patient care, this PRN Case Manager role in Medford is worth a look. You'll work Monday through Friday, 8:00 a.m. to 5:00 p.m., covering Medford and the surrounding area, with the flexibility that comes with a PRN schedule.

Requirements

• Graduate of an approved nursing program, currently licensed as an RN in the state of practice
• At least one year of RN experience
• Current CPR certification
• Valid driver's license with insured, reliable transportation for travel between locations

Responsibilities

• Conduct comprehensive nursing assessments covering physical, emotional, and psychosocial needs.
• Build and adjust individualized care plans as conditions change.
• Deliver direct skilled care, including wound care, medication management, IV therapy, and symptom management.
• Provide hands-on education and emotional support to patients and caregivers.
• Coordinate visit scheduling and documentation to keep everything compliant.
• On-call rotation, admissions, and urgent response may also come up as part of the role.
• Supervise LPNs/LVNs, home health aides, and other support staff.
