About The Position

Concerto Renal Services is seeking a PRN Dialysis Nurse to provide and supervise hemodialysis care in skilled nursing and/or long-term care facilities. The role is responsible for the dialysis unit and works collaboratively with the hemodialysis healthcare team, supervising dialysis technicians while also providing hands-on patient care. Care must follow organizational policies, procedures, and training, as well as applicable state and federal regulations.

Requirements

Graduate of an accredited school of Nursing.
Current state-appropriate or Compact RN License.
At least one year of nursing experience in any practice area.
At least three months of dialysis experience.
BLS Provider certification.
Successful completion of the Ishihara Color Blindness Test.
Ability to walk, stand, stoop, and bend for prolonged periods and lift up to 25 lbs.

Responsibilities

Provide direct hemodialysis care, perform assessments, administer medications, and educate patients about renal health, treatment, and self-care.
Delegate tasks to dialysis technicians, supervise their performance, and ensure compliance with clinical standards and regulations.
Monitor patient responses to dialysis, adjust treatment plans as needed, and document interventions and outcomes accurately.
Collaborate with physicians and healthcare team members to coordinate care, report critical lab results, and support continuity of treatment.
Maintain accurate electronic medical records and complete care plans on time.
Follow safety and regulatory protocols and take responsibility for the dialysis unit.
