About The Position

Unity Hospice is seeking a compassionate Primary Registered Nurse (RN) to join their team. This role serves as the lead case manager for patients in home and facility settings, partnering with float nurses and an interdisciplinary team to deliver high-quality, patient-centered care. The position is ideal for a nurse who values autonomy, teamwork, and meaningful patient relationships. During periods of lighter caseloads, the RN will support the team by assisting with patient visits and transitions, acting as a Float Registered Nurse as needed. This collaborative approach ensures all patients receive the highest level of care. The role is based in the Green Bay territory with occasional travel outside the territory.

Requirements

• Current unrestricted Registered Nurse licensure from the State of Wisconsin
• Minimum of 2 years RN experience required, equivalent nursing experience may be considered
• Valid Wisconsin Driver’s License, reliable transportation and proof of current automobile insurance coverage

Responsibilities

• Provide compassionate hospice care and case management
• Assess patient and family needs and adjust care plans accordingly
• Respond to urgent situations and initiate crisis care when needed
• Collaborate with physicians and interdisciplinary team members
• Educate and support families through the end-of-life journey
• Administer medications and document care in the EMR
• Delegate and guide LPNs and CNAs
• Manage daily priorities and assist with admissions
