Hamilton Urban Core Community Health Centre (HUCCHC) is a dynamic, community-governed primary healthcare organization dedicated to improving the health and well-being of individuals, families, and communities living within Hamilton's urban core. Through an interdisciplinary and client-centred model of care, HUCCHC provides comprehensive primary healthcare, health promotion, illness prevention, advocacy, and community development services to individuals experiencing complex health and social challenges. Hamilton Urban Core is committed to advancing health equity and addressing the social determinants of health. Our priority populations include individuals experiencing homelessness or housing instability, newcomers, immigrants and refugees, individuals and families living with low income, people living with mental health and substance use challenges, street-involved youth, and isolated seniors. The Physician will also support Refugee Health services through the Centre's specialized Refugee Transition Clinic. The Refugee Health Physician provides primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community with particular emphasis on care and services for clients with chronic and complex health conditions.
