About The Position

Coös County Family Health Services (CCFHS) is seeking compassionate, dedicated healthcare providers passionate about making a meaningful impact in the communities they serve. As a Federally Qualified Health Center (FQHC), CCFHS is committed to providing high-quality, accessible care to all patients, regardless of their ability to pay. For over 10 years, CCFHS has offered comprehensive office-based primary care services, including diagnosis and treatment of acute and chronic illnesses, preventive services, screenings, and health education. They also assess patient needs for social and nutrition services and coordinate referrals to specialty medical and behavioral health providers. CCFHS welcomes applications from Physicians, Nurse Practitioners, Physician Assistants, Dentists, Behavioral Health Providers, and other licensed clinicians dedicated to patient-centered care in a collaborative, team-based environment.

Requirements

  • New Hampshire-licensed Physicians (MDs), Nurse Practitioners (NPs), or Physician Assistants (PAs).
  • Commitment to delivering patient-centered care in a collaborative, team-based environment.
  • Passion for community health and serving underserved and rural populations.

Responsibilities

  • Providing high-quality, accessible care to all patients, regardless of their ability to pay.
  • Diagnosis and treatment of acute and chronic illnesses.
  • Preventive services, screenings, and health education guided by evidence-based practices.
  • Assessing patient needs for social and nutrition services.
  • Coordinating appropriate referrals to specialty medical and behavioral health providers.
  • Delivering patient-centered care in a collaborative, team-based environment.
