About The Position

Callen-Lorde is seeking a Primary Care Provider (NP/MD/DO) who specializes in HIV care to join their collaborative, multidisciplinary team. The role involves providing patient-centered healthcare, delivering high-quality, respectful care to a diverse patient population. Responsibilities include conducting patient screenings, examinations, diagnoses, and treatments across the lifespan, ensuring care is respectful, compassionate, unbiased, and consistent with evidence-based guidelines. Care is delivered in scheduled appointments with timely follow-up on patient needs, test results, and referrals. All care is documented promptly in the electronic health record (EPIC). The provider will also make appropriate internal and external referrals, work effectively within a team-based care model, and participate in quality improvement initiatives and case conferences. Ongoing professional development, including continuing medical education, is also expected.

Requirements

  • New York State Certification/Licensure (MD, DO, or NP)
  • Current NYS Registration and DEA Controlled Substances Registration Certificate
  • Current CPR Certification
  • Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease
  • Ability to work effectively with individuals from a wide range of backgrounds
  • Commitment to accessible, patient-centered primary care
  • Strong collaboration skills in a team-based model

Nice To Haves

  • Experience in community-based primary care preferred

Responsibilities
