About The Position

Central City Concern providers practice in flexible, community-based settings that bring care directly to patients. They provide low-barrier, trauma-informed primary care to individuals experiencing homelessness and complex health challenges. This role offers the opportunity to practice innovative, low-barrier medicine for underserved communities, work within a multidisciplinary team supporting whole-person care, and help patients build stability, health resilience, and long-term engagement in care.

Requirements

NP/PA Graduate of an accredited Nurse Practitioner Program or Physician Assistant program required of all Nurse Practitioner applicants.
If candidate is a Nurse Practitioner, graduate of an accredited school of nursing and has unrestricted licensure to practice medicine in Oregon or eligibility for such licensure.
Current Advanced Practice Registered Nurse (APRN) Licensure with National certification or Licensed to practice as a Physician’s Assistant in Oregon, with at least 2 years’ experience.
Demonstrated skills in primary care medicine experience preferred.
Must meet CCC privileging requirements as required by FTCA.
Must have full prescriptive powers in Oregon with a DEA number.
Must have current BLS certification.
Must pass a pre-employment drug screen, TB test, and background check.
This includes clearance by the DHS Background Check Unit.
Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
Must adhere to agency’s non-discrimination policies.
Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, lifestyles, and sexual orientations and treat each individual with respect and dignity.

Sufficient manual dexterity and physical ability to perform assigned tasks with reasonable accommodation as needed.
Ability to manage time and meet deadlines.
Knowledge of de-escalation methods or willingness to be trained in these methods.
Comfort working in an independent clinical practice with on and off-site support from medical and administrative supervisors.

Nice To Haves

Wound care experience highly preferred.
Spanish language fluency preferred.
Valid driver's license preferred.
Experience in primary care or mobile medical practice.

Responsibilities

Diagnose and treat acute and chronic health conditions
Provide integrated care addressing physical health, mental health, and substance use disorders
Prescribe medications, including medications for opioid use disorder using a harm reduction approach
Deliver wound care and preventative services common in homeless medicine
Work as a member of the primary care team to deliver comprehensive, patient-centered services.
Take appropriate health histories, perform physical exams, review vitals, labs, other studies, prescribe medications, perform procedures, review outside medical records and other forms of treatment as necessary and appropriate to the standard of care being provided in the setting for both acute and chronic health concerns.
Triage, diagnose, manage minor and/or acute illnesses and initiate treatment.
Along with the care team, support patients in achieving health resiliency and engagement by utilizing techniques such as shared decision making, motivational interviewing, continuity relationship building, health education, and preventive care appropriate to patient characteristics.
Timely completion of appropriate medical records.
Attend all mandatory CCC training in a timely manner.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.
Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access.
This position will provide after-hours phone coverage on scheduled rotation with colleagues.
Support pharmacy-based collaborative drug therapy management programs and provide medical visits to support clients receiving treatment.
Coordinate with other CCC programs supporting patients in a continuum of services such as Hepatitis C treatment, withdrawal management, residential treatment for substance use disorders, specialty mental health, and overdose prevention programs
Identify opportunities for improvement in medical workflows and logistics.
Participate in review and collection of performance and quality Indicators; review and contribute to continuous improvement opportunities and plans.
Participate in and, at times, lead provider education sessions on a variety of clinical topics, including helping train and onboard other medical providers and staff.
Participate in compliance, quality assurance, and quality improvement activities as directed.
Collaborate with the care team to ensure optimal performance on team-based performance metrics.
Perform other duties as assigned, including work at other CCC clinical sites and/or off-site at community events.
