About The Position

The Union Health Center is a nationally recognized, innovative primary and specialty care practice in a state-of-the-art facility in the Chelsea neighborhood in New York City. The Center provides multidisciplinary team-based care and is an NCQA-certified level 3 Patient-Centered Medical Home. We seek dedicated Board-certified/eligible primary care internists and family practitioners for our growing practice, primarily serving union members, retirees, and their families. The ability to communicate verbally in Spanish is a plus.

Requirements

  • Board Certified
  • 2 to 5 years’ experience post board certification preferred
  • Meet the criteria for active medical staff membership
  • Strong interpersonal, oral, written communication and collaborative skills.
  • Computer literate.

Nice To Haves

  • Language proficiency in Spanish preferred.

Responsibilities

  • Delivers professional services within the Center.
  • Elicits and records information regarding patient's medical history electronically.
  • Examines patient to determine general physical condition.
  • Orders or executes various tests, analyses, and diagnostic images to provide information on patient's condition.
  • Analyzes reports and findings of tests and of examination, and diagnoses condition.
  • Administers or prescribes treatments and drugs.
  • Performs telemedicine including phone calls and video calls to patients and records findings electronically.
  • Completes charting in a timely manner.
  • Inoculates and vaccinates patients to immunize patients from communicable diseases.
  • Advises patients concerning diet, hygiene, and methods for prevention of disease.
  • Provides lab and diagnostic test results to patients in a timely manner.
  • Reports outbreak of contagious diseases to governmental authorities.
  • Refers patients to medical specialist or other practitioner for specialized treatment.
  • Performs minor procedures.
  • Contributes to the development of policies and standards of patient care by participating in committees.
  • Complies with the Union Health Center’s policies & procedures related to privacy and security of identifiable patient information (PHI), as explained in the HIPAA (Health Insurance Portability & Accountability Act) training.
  • Supports the stated goals and objectives of the Patient Centered Medical Home by working cooperatively with all clinical and non-clinical staff to develop procedures that enhance the Health Center’s team-based approach to patient care.
