About The Position

WelbeHealth is seeking a Primary Care Physician to provide community-based, comprehensive care to vulnerable seniors through its value-based PACE program. The role supports long-term relationships with a smaller patient panel and works within a mission-driven interdisciplinary team. Physicians see an average of eight patients per day and do not conduct hospital rounds. Work includes home visits and a rotating, phone-only on-call schedule a few nights or weekends per month. The position offers regular weekday hours, weekends off except for rotating on-call coverage, and opportunities for growth toward a medical director role. Compensation consists of base salary plus bonus, with the final amount based on experience and relevant qualifications. Employees must comply with WelbeHealth’s vaccination policy and infection-control and PPE standards.

Requirements

Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
Master's or doctorate degree.
Active state physician license without restrictions.
State CDS certification and DEA registration.
Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants.
Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter.
Physicians in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics are sought.

Board certification preferred.
CPR and First Aid certification upon hire preferred.

Responsibilities

Perform comprehensive in-person assessments.
Lead and partner with the interdisciplinary team (IDT) to develop a personalized care plan for each participant.
Coordinate discharge for acute, short-term, and long-term placement.
Provide care through home visits and participate in a rotating, phone-only on-call schedule.
