Requirements

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
  • Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working.
  • Board Eligibility is required.
  • Must have a current DEA number for schedule II-V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment.
  • Competency in Availability and Accessibility for patients to build trust.
  • Demonstrate Service Orientation, providing care as they would want for a family member.
  • Stay updated on Evidence Based Medicine, including clinical, nutritional, and lifestyle-based interventions.
  • Willingness to continuously work to develop and improve Physician Leadership skills.
  • Willingness to work towards achieving high quality outcomes and quality measures, and continuously improve practice and center operations.
  • Competently and compassionately influence patients, teams, and self to achieve the best outcomes.
  • Practice Self-Care (physically, mentally, socially, and spiritually).
  • Proficient in Microsoft Office Suite products (Word, Excel, PowerPoint, Outlook) and other relevant software.
  • Ability and willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes.
  • Fluency in English, verbal and written.
  • Requires use and exercise of independent judgment.
  • Flexible to work evening, weekends and/or holidays as needed.

Nice To Haves

  • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.
  • Fluency in other languages may be required for certain center jobs.

Responsibilities

  • Demonstrate accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients.
  • Act as a health coach, helping patients set and achieve short and long-term health goals through partnership and follow-up.
  • Simplify and prioritize care to make behavior change actionable for patients and lead care teams towards performance goals.
  • Independently provide care for patients with acute and chronic illnesses encountered in older adult patients.
  • Take full accountability for patient care and outcomes, seeking consultation from specialists when needed while remaining involved in detailed patient care.
  • Engage with hospitalists when a patient is hospitalized, regardless of whether the hospitalist works for ChenMed.
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
  • Lead their care team, consisting of a care promoter (medical assistant), care facilitator, and care coordinator, for patients able to come to the office.
  • Engage with the transitional care team, case managers, acute and transitional-care physicians, and other available resources for patients unable to come to the office (in hospital, SNF, LTC, or homebound).
  • Take an active role in the management of their center and cover for other providers as needed.
  • Take an active role as needed in recruiting patients for the center and additional providers for the company.
  • Supervise, collaborate with, participate with, or function within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remain accountable for the APP's actions.
  • Perform other duties as assigned and modified at manager’s discretion.
