About The Position

Dedicated Senior Medical Centers of Florida, LLC is seeking innovative and entrepreneurial-minded Primary Care Physicians to join their rapidly expanding team. This role involves independently providing care for patients with acute and chronic illnesses common in older adults, taking full accountability for patient care and outcomes. The physician will engage with hospitalists when their patients are hospitalized and will be responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. For patients unable to come to the office (in hospital, SNF, LTC, or homebound), the physician will collaborate with the transitional care team, case managers, and other available resources. The role also includes leading Super Huddle (SH) and Transforming Care Meeting (TCM) weekly, and supporting Center Manager/Center General Manager if the center or market clinical leader is unavailable, under the guidance of the Market Chief Medical Officer. The physician may also work at other Dedicated Senior Medical Center sites in Orange County.

Requirements

  • Medical Degree or foreign equivalent
  • Board Certified/Board Eligible (BC/BE) in Family Medicine or Internal Medicine
  • Must possess a Florida State Medical license
  • Must possess a Drug Enforcement Administration (DEA) license

Responsibilities

  • Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
  • Takes full accountability for patient care and outcomes and appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
  • Engages with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
  • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, engages with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
  • Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly, as well as supports Center Manager/Center General Manager if the center clinical leader and/or market clinical leader is not available, based on guidance from Market Chief Medical Officer.
  • May work at other Dedicated Senior Medical Center sites in Orange County.
