About The Position

Primary Care Physician to join our team full-time at CenterWell Primary Care. This role is ideal for a compassionate, experienced clinician who succeeds in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations. Requirements

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of practice.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
• 2 years' experience in value-based care or managing high-acuity geriatric patients.
• Commitment to improving patient experience and outcomes.
• If hired, you will be required to be screened for TB.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.

• Participate in clinical quality improvement projects.
• Work with interdisciplinary teams to ensure holistic care.
• Contribute to projects and innovations in care delivery.
