Primary Care Physician
About The Position
Conviva Senior Primary Care, a subsidiary of Humana Inc., is recruiting a full-time primary care physician for our Ormond Beach, Florida center. We have a unique, value-based care model and we are experts in senior-focused primary care. As a Primary Care Physician, you will report to our Chief Medical Officer/Regional Medical Director. The Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Begins to influence department's strategy.
Requirements
- Doctor of Medicine (MD) or Osteopathic (DO)
- Board certified or board eligible in Family Medicine or Internal Medicine
- Valid, unrestricted license (or willing to obtain) in the state of desired employment (Florida)
- Current, unrestricted DEA license
Nice To Haves
- Geriatrics specialty would be ideal
- Minimum of two to five years directly applicable experience preferred
- New graduates will be considered
- Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment
- Knowledge of Medicare guidelines and coverage
- Knowledge of HEDIS quality indicators
- Bilingual (English and Spanish) is a plus
