About The Position

ZoeRVA Health is transitioning its insurance-based medical practice (Zoe Primary Care) to a direct primary care model named Zoe Direct. This new practice will focus on preventive care and wellness, aiming to empower individuals in managing their health proactively. The approach emphasizes guidance on lifestyle improvements such as nutrition, physical activity, stress management, healthy habits, sleep, and relationships. Recognizing that each patient is unique, Zoe Direct will offer personalized care to equip patients with the tools for healthier lives. As a new organization, this role presents an opportunity to shape care delivery, influence clinic culture, and build systems for long-term community impact.

Requirements

  • At least 5 years of clinical experience; in a primary care setting preferred
  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution
  • Board certification in Family Medicine, Internal Medicine, or Med-Peds
  • Active medical licensure in VA

Nice To Haves

  • Lifestyle Medicine board certification preferred

Responsibilities

  • Provide comprehensive primary care services to a diverse patient population; conduct thorough assessments, diagnoses, and treatment plans for patients across all age groups
  • Diagnose and manage acute and chronic conditions with evidence-based protocols
  • Provide preventive care including screenings, immunizations, patient education, and lifestyle counseling
  • Collaborate with behavioral health and specialty providers to ensure continuity of care
  • Refer patients appropriately and follow up on external consultations and diagnostics
  • Engage in practice-wide initiatives to improve patient outcomes and operational efficiency
  • Monitor and respond to population health metrics and patient satisfaction data
  • Maintain accurate and timely documentation; uphold HIPAA and ethical standards in all patient interactions
  • Contribute to clinic development by participating in quality improvement initiatives; collaborate with leadership to refine procedures that enhance patient experience, staff efficiency, and clinical outcomes
