Primary Care Physician Opportunity – Bakersfield, California
About The Position
HealthPlus Staffing is seeking a Board Certified or Board Eligible Family Medicine or Internal Medicine Physician for a full-time opportunity in Bakersfield, California. The physician will provide comprehensive primary care to an older adult population within a PACE (Program of All-Inclusive Care for the Elderly) model. This role is designed for a physician who enjoys managing complex medical needs while working closely with an interdisciplinary healthcare team. The incoming physician will play an important role in assessing participants, managing acute and chronic conditions, developing individualized treatment plans, and coordinating care across multiple disciplines.
Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Active California medical license
- Active DEA license
- Must not be restricted from seeing Medi-Cal or Medicare patients
- Minimum of one year of documented experience working with a frail or elderly population
- Current CPR/BLS certification
- Strong clinical assessment and chronic disease management skills
- Ability to effectively manage medically complex older adults
- Strong communication and interpersonal skills
- Ability to work collaboratively within an interdisciplinary healthcare team
- Commitment to compassionate, patient-centered care
Nice To Haves
- Geriatric medicine experience or additional qualifications in Geriatrics preferred
Responsibilities
- Provide comprehensive primary care and medical management for PACE participants.
- Conduct initial and ongoing comprehensive medical assessments.
- Obtain medical histories and perform physical examinations.
- Diagnose and manage acute and chronic medical conditions.
- Order and interpret laboratory and diagnostic testing.
- Develop and update individualized treatment and care plans.
- Manage medications and adjust treatment when clinically appropriate.
- Provide preventive care, health screenings, and health education.
- Evaluate changes in patients' physical, functional, and cognitive status.
- Coordinate specialty consultations and additional medical services when needed.
- Collaborate with the interdisciplinary care team to address medical, functional, and psychosocial needs.
- Educate patients and families regarding diagnoses, treatment plans, medications, and disease management.
- Coordinate care when assigned patients require hospitalization or skilled nursing care.
- Maintain accurate and timely clinical documentation.
- Participate in quality assessment and performance-improvement activities.
- Serve as a source of medical and geriatric expertise for the care team.
- Provide supervisory collaboration with Nurse Practitioners and/or Physician Assistants as applicable.
