One Medical Seniors is a network of primary care practices dedicated to providing personalized care for adults on Medicare and those needing extra attention. Practices feature smaller panel sizes, no billing or coding responsibilities, and opportunities for systemic change in healthcare delivery. One Medical Seniors aims to restore humanity to healthcare for both patients and team members. The company is seeking Primary Care Physicians for their growing outpatient practices in Charlotte, NC. Team Physicians will collaborate with Medical Directors, playing a key role in care delivery and practice innovation. Requirements

  • At least 2 years of primary care experience required.
  • Currently licensed or ability to obtain licensure in the state of North Carolina
  • The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups.
  • Can thrive in a fast-growing, mission driven organization focused on using data to improve patient outcomes.
  • Board Certification in Internal or Family Medicine required.

Responsibilities

  • Provide best in class comprehensive primary care in an outpatient setting.
  • Partner with the care team and leverage One Medical Seniors coaching and integrated behavioral health models to engage our geriatric patient population in care and drive behavior change.
  • Utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population.
  • Willing to obtain additional state licensure and credentialing for One Medical virtual primary care in additional states
