About The Position

Redwood Coast PACE is seeking a Primary Care Physician to provide comprehensive care for participants who wish to remain living in their homes and communities. The program offers medical, social, nutritional, rehabilitation, and support services to eligible adults, aiming to keep them healthy and independent. The physician will work collaboratively with an Interdisciplinary Team (IDT) and other healthcare providers to ensure continuity of care across various settings, including the PACE clinic, participants' homes, residential care facilities, skilled nursing facilities, and hospitals. Responsibilities include developing and implementing individualized Plans of Care, overseeing quality improvement, and providing medical leadership.

Requirements

  • Graduate of an accredited School of Medicine.
  • Board certified in Internal Medicine or Family Practice.
  • Board certification granted by the American Boards of Internal Medicine and Family Practice.
  • Two (2) years of experience working with a frail or elderly population required, preferably in a hospital or long-term care facility.
  • Licensed by the California Medicine Board to practice medicine in California.
  • Board certification in internal medicine or family practice plus current Drug Enforcement Administration (DEA) Registration Number required.
  • Pre-employment physical, and PPD skin test (TB screening).
  • Successful completion of a background check.
  • Medical knowledge and clinical skills necessary to provide comprehensive, interdisciplinary care to frail, elderly, and disabled adults, including the ability to manage complex clinical conditions while promoting participant dignity, independence, and quality of life.
  • Knowledge and experience working with physical, social, and mental health programs operating within a licensed health facility or clinic.
  • Demonstrates respect for and promotes participants’ rights including dignity, self-determination, access to care, confidentiality, and independence.
  • Ability to function effectively within a multi-service, community-based nonprofit agency.
  • Ability to comply with program requirements and applicable government regulations.
  • Strong verbal and written communication abilities; able to convey information clearly and professionally to staff, participants, families, and external partners.

  • The position requires the ability to sit, stand, walk, bend, stoop, reach, and move throughout clinic and community-based care settings for extended periods.
  • The employee must be able to travel between work locations and enter and navigate participants’ homes, hospitals, skilled nursing facilities, assisted living facilities, and other care settings.
  • The position requires sufficient visual, auditory, and manual dexterity abilities to conduct physical examinations, review medical records, use medical equipment and computer systems, and document participant care.
  • The employee may occasionally need to assist with or respond to participant mobility or safety needs and may be exposed to infectious diseases, bodily fluids, and other conditions commonly encountered in healthcare settings.

Nice To Haves

  • Certificate of Added Qualifications in Geriatric Medicine (CAQ) preferred.
  • Experience working as part of an IDT preferred.
  • Valid California Driver’s License and personal auto liability insurance preferred.
  • Personal vehicle in good driving/operating condition, insured for State minimum liability requirements that can be used for the applicable job functions noted above.
  • COVID-19 and Influenza (flu) vaccinations preferred.

Responsibilities

  • Performs initial assessment, including comprehensive history and physical, of assigned participants at intake, and at least every 6 months in cooperation with family member and/or caregiver and Interdisciplinary Team (IDT) members.
  • Develops and implements a therapeutic Plan of Care at least every 6 months and when participant conditions change.
  • Monitors Plan of Care on an ongoing basis.
  • Presents and coordinates the Plan of Care with the IDT.
  • Helps ensure that participants experience a smooth transition into Redwood Coast PACE by coordinating with previous medical providers.
  • Provides evaluation, treatment, and ongoing monitoring of episodic illness.
  • Refers participants to medical specialist as indicated.
  • Provides oversight of hospitalized participants (in collaboration with a hospitalist as appropriate) including, but not limited to providing primary care responsibilities for management, providing updates to the team on a regular basis, judiciously utilizing medical specialty consultants, and coordinating discharge planning with the IDT.
  • Provides primary care to participants in accordance with professional standards and applicable laws and regulations.
  • Provides care in a variety of settings, including the clinic, skilled nursing facilities, participants’ homes, assisted living facilities, hospice inpatient units, and hospitals, as appropriate to meet participant care needs.
  • Maintains timely and effective communication and coordination with hospital, skilled nursing, hospice, and assisted living staff to support continuity and quality of participant care.
  • Collaborates with internal Nurse Practitioners by providing consultation and medical leadership.
  • Participates as an active member of the IDT by attending IDT meetings, family meetings and clinical case conferences, consulting with Nurses and other team members as medical care issues arise, and attending team and other meetings, as applicable.
  • Provides on-call medical coverage to participants after normal Clinic business hours.
  • Participates actively in Redwood Coast PACE Quality Assessment and Performance Improvement (QAPI) activities.
  • Works closely with Redwood Coast PACE Nurse Practitioners and Registered Nurses to provide clinical care.
  • Assigns, orders, and delegates appropriate nursing interventions to Registered Nurses, as determined by the participant’s Plan of Care.
  • Participates in primary care program development and implementation as determined by the needs of Redwood Coast PACE.
  • Understands fiscal issues and is appropriately involved in meeting budget targets.
  • Complies with safety policies and procedures, including identifying and immediately reporting unsafe acts or conditions to the Chief Medical Officer and Director of Redwood Coast PACE.
  • Takes necessary measures to ensure a safe environment.
  • Continually seeks better ways for delivering services and communicating with participants.
  • Understands the importance of community involvement and participates as appropriate in activities that link Redwood Coast PACE to the community.
  • Effectively collaborates with staff peers and contractors to meet Redwood Coast PACE goals and further success.
  • Complies with all policy and procedures of Redwood Coast PACE.
  • Participates in the process of teaching, training, and mentoring student interns, and volunteers working within the program area.
  • Acts in a manner consistent with agency values.
  • Additional duties may be assigned, as needed, that align with the responsibilities of the position and support the vision, mission, and values of the organization.
