About The Position

The PCP in this organization is accountable for patient outcomes, providing strong clinical care, and ensuring cost-effectiveness for a panel of up to 450 patients. They act as a health coach, partnering with patients to set and achieve health goals, and simplifying complex health information to facilitate actionable behavior change. The organization is outcomes-focused and value-based, with key metrics regularly measured to support PCP success and partnership status. PCPs are expected to champion a positive culture of love, accountability, and passion within their centers. The PCP will function independently and collaboratively with other healthcare professionals, working closely with managers and medical directors to ensure compliance, participate in risk and quality management programs, and attend required meetings. Adherence to departmental goals, standards of performance, regulatory compliance, quality patient care, and policies and procedures is essential.

Requirements

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
  • Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working.
  • Board Eligibility is required.
  • Must have a current DEA number for schedule II-V controlled substances.
  • Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software as used in the company.
  • Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes.
  • Fluency in English, verbal and written.
  • This job requires use and exercise of independent judgment.
  • Flexible to work evening, weekends and/or holidays as needed.

Nice To Haves

  • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.
  • Once Board certified, PCP will maintain board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required.
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment.

Responsibilities

  • Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
  • Takes full accountability for patient care and outcomes, appropriately seeking consultation from specialists while remaining involved in the detailed care of the patient.
  • Engages with hospitalists when their patients are hospitalized.
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
  • Leads their care team (care promoter, care facilitator, care coordinator) for patients able to come to the office.
  • Engages with the transitional care team, case managers, acute and transitional-care physicians, and other resources for patients unable to come to the office (in hospital, SNF, LTC, or homebound).
  • Takes an active role in the management of their center and covers for other providers as needed.
  • Takes an active role as needed in recruiting patients for the center and additional providers for the company.
  • Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP.
  • Performs other duties as assigned and modified at manager’s discretion.
