About The Position

The Physician serves as a health-care professional and able to handle a variety of health-related problems. The Physician work assignments involve moderately complex to complex issues where the analysis of situations or data requires an in-depth evaluation of variable factors. You will work on-site at our Chesapeake clinic. You will report to the Associate Medical Director for the Tidewater Region. We are looking for a Primary Care Physician to join our team full-time at CenterWell Primary Care. Requirements

• MD or DO from an accredited medical school.
• Active, unrestricted medical license in the state of practice.
• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
• If selected for this role, you will be required to be screened for TB

Nice To Haves

• 2 years' experience in value-based care or managing high-acuity geriatric patients.
• Bilingual English/Spanish.
• Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency.
• Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
• Coordinate referrals and collaborate with specialists.
• Participate in clinical quality improvement projects.
• Work with interdisciplinary teams to ensure holistic care.
• Contribute to projects and innovations in care delivery.
