Requirements
- Active New Jersey Nurse Practitioner license
- National board certification
- DEA registration or eligibility
- Strong clinical assessment and decision-making skills
- Excellent communication and interpersonal skills
- Ability to work independently while collaborating within a multidisciplinary team
Nice To Haves
- Minimum of two years of Nurse Practitioner experience preferred
- Experience in hospital medicine, internal medicine, family medicine, geriatrics, critical care, or management of medically complex patients
- Experience caring for hospitalized adult patients
- Bilingual skills are a plus
Responsibilities
- Perform comprehensive patient evaluations and follow-up visits
- Manage medically complex adult patients with multiple chronic conditions
- Develop and implement individualized treatment plans
- Coordinate care with physicians, specialists, nursing staff, and hospital teams
- Educate patients and families regarding diagnosis, treatment, and ongoing care
- Document patient encounters accurately within the electronic medical record
- Participate in interdisciplinary rounds and quality improvement initiatives
- Collaborate with care teams to ensure safe discharge planning and continuity of care
