At Open Door Community Health Centers (ODCHC), we provide every patient with compassionate, whole-person care through our Medical, Dental, Behavioral Health, and Pharmacy services across California's North Coast. We are looking for Primary Care Nurse Practitioners (NP) and Physician Assistants (PA) to join our integrated resident care team at Eureka CHC, in Eureka, California. This is an outpatient practice with excellent work-life balance, part of a supportive, success-driven program with a high-performance PCMH model. The role is part of an Advanced Practice Clinician (APC) Residency Program which consists of a 12-month residency, followed by a 12-month fellowship for those intending to remain with ODCHC. Established in 2016, the program provides intensive training in the clinical complexity of rural family practice within both a safety-net setting and a high-performance, patient-centered medical home (PCMH) model of primary care. The program is designed for newly graduated NPs and PAs committed to serving as primary care providers (PCPs) in community health centers and other safety-net settings. Residents are trained in Open Door’s model of high-performance health care, including advanced access scheduling, planned care, the chronic care model, integrated behavioral health and primary care, and team-based collaboration, supported by expert use of health information technology and the electronic health record (EPIC).
