About The Position

Infinite Medical P.C. is hiring a Primary Care Nurse Practitioner or Physician Assistant to provide on-site primary care to residents in long-term care settings. The part-time position is based in Stoughton, Massachusetts, and is suited to a provider who values meaningful patient relationships and collaborative care. Infinite Medical P.C. is a nationwide network of advanced practice providers and specialty clinicians serving residents in skilled nursing and long-term care facilities, partnering with MedElite Healthcare Management Group to provide compassionate, personalized care.

Requirements

Hold an active, unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
Hold a state-specific license in good standing in Massachusetts.
Have at least 6 months of experience in long-term care or a skilled nursing facility.
Have strong communication, clinical, and critical-thinking skills.
Have a passion for elder care and person-centered healthcare.
Meet the stated physical demands, including prolonged standing and walking, occasional lifting up to 20 pounds, computer work, lifting or moving patients, and prolonged sitting.

Responsibilities

Provide primary care services to residents in skilled nursing and long-term care facilities.
Perform physical examinations and medical history assessments.
Diagnose and treat chronic and acute health conditions.
Order and interpret diagnostic tests and laboratory results.
Develop and adjust individualized care plans.
Prescribe medications and monitor treatment effectiveness.
Collaborate with facility staff and interdisciplinary teams.
Educate patients and families about treatment options and care goals.
Ensure a safe, respectful, and supportive environment for residents.
