About The Position

Open Door Community Health Centers (ODCHC) is seeking Primary Care Nurse Practitioners (NP) and Physician Assistants (PA) to join their integrated resident care team at Eureka CHC in Eureka, California. ODCHC provides compassionate, whole-person care across Medical, Dental, Behavioral Health, and Pharmacy services on California's North Coast. The Advanced Practice Clinician (APC) Residency Program is a 12-month program, followed by an optional 12-month fellowship for those intending to stay with ODCHC. Established in 2016, the program offers intensive training in the clinical complexities of rural family practice within a safety-net setting and a high-performance, patient-centered medical home (PCMH) model. It is designed for newly graduated NPs and PAs committed to serving in community health centers and safety-net settings. The training focuses on ODCHC's model of high-performance healthcare, incorporating advanced access scheduling, planned care, the chronic care model, integrated behavioral health, team-based collaboration, and expert use of health information technology and the electronic health record (EPIC).

Requirements

• Diploma from an accredited program with credentialing through the American Nurses Association or Physician Assistant Association within the last 18 months.
• Current, unrestricted California license.
• Current DEA registration.
• Current Board Certification in specialty of practice (or board eligible if a newer graduate).
• Current CPR/BLS certification.
• Valid California driver's license with an insurable driving record.
