Requirements

  • Board Certified or Board Eligible in Internal Medicine, Geriatric Medicine or Family Medicine
  • Active and unrestricted Arizona medical license or ability to obtain prior to start date
  • Current and unrestricted DEA certificate or ability to obtain prior to start date
  • 3+ years of primary care physician experience

Nice To Haves

  • Experience practicing in a value-based care setting
  • Strong familiarity with HEDIS measures

Responsibilities

  • Deliver an enhanced level of care for patients with high hospitalization rates, frequent ER utilization, or complex medical and social needs.
  • Provide dedicated care to Medicare Advantage patients for 60-90 days following hospital discharge, including addressing social determinants of health, referral and medication management, care gap assessments, and more.
  • Manage primary care for complex patients while partnered with an interdisciplinary care team.
  • Focus on patient needs with 60 minutes per appointment.
  • Meet patients where they are, with clinic visits, virtual visits, and potential home visits with the Case Manager.
  • Support patients and caregivers in confidently navigating the healthcare system and available resources.
  • Improve clinical outcomes, patient experience, and quality of life through collaborative, team-based care.
