About The Position

Ellis Medicine is seeking a Primary Care Advanced Practitioner to join our primary care practice in Glenville, NY. This is an excellent opportunity for a Nurse Practitioner or Physician Assistant who enjoys providing comprehensive care to patients across the lifespan and wants to become part of a collaborative, community-focused primary care practice. Our Glenville practice serves approximately 12,000 patients and cares for both children and adults. The practice offers an established patient population, dedicated clinical support, and an opportunity for an APP to develop meaningful, long-term relationships with patients and families.

Requirements

• Current NP or PA certification
• Licensed (or eligible) to practice in New York State
• Strong clinical judgment and communication skills
• Ability to work collaboratively within a multidisciplinary team
• Commitment to high-quality, patient-centered care

Nice To Haves

• Primary Care experience preferred but not required
• New graduates are welcome to apply

Responsibilities

• Provide comprehensive primary care to pediatric, adolescent, and adult patients
• Diagnose, treat, and manage acute and chronic medical conditions
• Establish and maintain long-term relationships with patients and families
• Perform preventive health examinations and routine health screenings
• Manage chronic conditions and coordinate appropriate specialty care
• Collaborate with nursing and clinical support staff to provide efficient, patient-centered care
• Coordinate care with Ellis Medicine specialists and other members of the patient's care team
