About The Position
Revere Medical is seeking an experienced Primary Care Advanced Practitioner (NP or PA) to join their established Primary Care team in Raynham, MA. Requirements
- Graduate of an accredited NP or PA program; current certification and licensure
- At least one year of APP experience, preferably in primary care
- Strong clinical skills, collaborative mindset, and proficiency with EMR systems
Responsibilities
- Provide comprehensive care to a diverse patient population
- Conduct thorough physical exams, diagnose medical conditions, and develop personalized treatment plans
- Promote health education and disease prevention
- Order and interpret diagnostic tests and manage chronic conditions
- Collaborate with specialists and other healthcare providers to ensure coordinated care
- Maintain accurate up-to-date medical records and patient charts
- Stay current with medical advancements and best practices in Primary Care
