About The Position

At At Home Harmony, we are redefining how seniors receive care by bringing high-quality, compassionate medical and personal services directly into the home. Based in Richmond, we're growing rapidly across Virginia and beyond. Our integrated care model combines home-based primary care, long-term care pharmacy, advanced dementia support, and in-home personal care—helping medically complex seniors remain safe, comfortable, and well-supported in their homes. We are seeking a compassionate, adaptable, and highly motivated Nurse Practitioner or Physician Assistant who is passionate about improving the lives of older adults living with dementia and complex chronic conditions.

Requirements

  • Minimum of three years of experience as an APP providing care within assisted living, memory care, and senior living communities.
  • Strong clinical expertise in geriatrics, dementia care, cognitive assessment, chronic disease management, and caregiver support.
  • A passion for delivering high-quality, relationship-centered care to older adults and their families.
  • The ability to thrive in a high-volume, fast-paced environment while maintaining exceptional clinical quality and patient satisfaction.
  • Excellent time management, organizational, and documentation skills.
  • Strong professional boundary-setting abilities with patients, families, caregivers, and community partners.
  • Exceptional communication and relationship-building skills, including the ability to navigate complex conversations related to goals of care, caregiver stress, and end-of-life planning.
  • A collaborative, team-oriented approach to interdisciplinary care and program development.
  • Comfort with telehealth, EMRs, AI-enabled documentation tools, and other healthcare technologies.
  • Adaptability and flexibility to help build and grow At Home Harmony's innovative dementia care program.
  • Current unrestricted VA state license as a Nurse Practitioner or Physician Assistant
  • Nationally board certified, current
  • Current BLS required
  • Active Prescriptive authority and DEA
  • Valid drivers license with clean driving record

Nice To Haves

  • The successful candidate will work closely with a dedicated Dementia Care Navigator and interdisciplinary team while helping develop best practices, workflows, and care pathways.
  • As the program grows, there is potential for advancement into a leadership role supporting the training and mentorship of future dementia-focused providers.

Responsibilities

  • Deliver medical care in the patient's home. This involves driving or riding in your vehicle to the place of service.
  • Diagnose, manage and treat complex chronic illnesses.
  • Analyze test data to diagnose, treat and arrange appropriate follow up for the patient to ensure continuity of care.
  • Educate patients and families on how to manage their health and well-being.
  • Effectively navigate difficult conversations related to end-of-life issues and goals of care.
