About The Position

OLV Human Services is seeking a dedicated and compassionate Preventive Services Caseworker to join their dynamic team. In this vital role, you will work directly with families to support and strengthen their ability to provide safe, stable, and nurturing environments. This is a full-time position.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, Criminal Justice or other related field.
  • A minimum of one year paid experience working with children and families is required.
  • NYS Driver’s License.
  • Must have a safe, reliable vehicle.
  • Computer skills.
  • Knowledge of electronic record system.

Nice To Haves

  • Experience with case management providing in-home or community services is preferred.

Responsibilities

  • Develop and maintain regular contact and therapeutic rapport with each family, with face-to-face contact in the family home occurring regularly (minimum two times a month with the caretaker and all children).
  • Communicate and collaborate with stakeholders (Case Managers, Physicians, School/Education staff, CSE, Health Home Care Managers, Drug Treatment Court, Family Court, Juvenile/PINS, Community Providers) to monitor service provision and ensure effective communication.
  • Complete thorough and accurate family/children progress notes, court/permanency reports, court summaries, Individual Service plans, Family Assessment Service Plan (FASPs), monthly summaries, comprehensive assessments, and other case-specific documentation in accordance with regulatory requirements in a timely manner.
  • Teach, model, and evaluate effective parenting and family management techniques, including parenting skills, behavioral management, and appropriate discipline.
  • Assist parents/caretakers in addressing safety, behavioral, educational, and psychological needs, and provide crisis intervention as needed.
  • Ensure all court-mandated and/or recommended services are offered, utilized, and maintained via referral/linkage or provided directly to minimize risk and enhance family cohesiveness and well-being.
  • Assist with transportation for children and/or families to services/meetings/court.
  • Comply with agency policies, federal and state regulations.
  • Perform other job-related duties as assigned by Supervisor.
