About The Position

The Prevention Navigator will assist the Care Manager and Care Navigator with all aspects of care coordination for SAMHSA HOUSE 2.0 clients. This includes completing initial enrollment (all required SAMHSA and program forms/assessments), promoting compliance with appointments and treatment regimens, and encouraging self-sufficiency. The role involves assisting clients to access services such as primary care, substance use treatment, mental health counseling, transportation, intensive care management, housing, and vocational/educational resources, through phone calls to clients and providers, and home visits when needed. The Prevention Navigator will also participate in outreach events as needed as well as field-based client outreach.

Requirements

  • High school diploma/GED required
  • Demonstrated experience providing HIV health education and risk reduction to MSM/Transgender communities of color
  • Demonstrated understanding of PrEP/PEP and HIV treatment in communities with barriers to accessing care
  • Strong socio-cultural identification with the priority populations of this service category
  • Knowledge of issues pertaining to homelessness, substance use, harm reduction, mental illness, transgender healthcare, and LGBT individuals

Nice To Haves

  • Bachelor's degree in human services, Public Health preferred
  • Good computer skills (Word, Excel, Outlook, etc)
  • Strong organizational skills and verbal/written communications skills

Responsibilities

  • Provide Outreach and Engagement to target population
  • Enrolling clients to The HOUSE 2.0 Program
  • Support case manager in provision of case management services
  • Follow up with the client and the referral provider to ensure that appointments are kept and that the patient is re-engaged in care and referred to needed treatment/prevention.
  • Conduct rapid HIV and Hep C testing
  • Perform various administrative duties, including updating the client list and outreach logs; assisting with outreach calls, and scheduling intakes and follow ups.
  • Maintain essential data for reporting and program evaluation in SPARS, eICare, including program enrollment, progress notes, care plans, referral activity, and updating contact information.
  • Complete UND enrollments and PrEP referrals as determined by client need
  • Promote sexual health and wellness using risk neutral approaches and evidence-based interventions
  • Build and maintain resource lists for external services and emergency services (i.e., housing, domestic violence, food pantry)
  • Participate in scheduled supervisory sessions with the Program Coordinator, Clinical Supervisor, and other Departmental Leaders as needed
  • Participation in youth community events, such as kiki balls, LGBT events, testing events, etc.
  • Conduct client/field-based outreach as needed
  • Attend IDT meetings and daily huddles and complete updated info by due date in preparation for meeting.
  • Facilitate groups and EBIs and complete documentation within 48 hours
  • Participate in reoccurring interagency meetings, as well as external community meetings to improve integrated care and maintain relationships with community partners
  • Participate in department activities such as new staff onboarding, YPS Pride Committee, CQI efforts, etc
  • Participate in Housing Works advocacy events (minimum 3 in person per year, 4 via other mode July-June)
  • Perform other duties as assigned
