Primary Care HIV Prevention (PCHP) is a U.S. Health Resources and Services Administration funded program aimed at decreasing the risk of HIV transmission, focusing on supporting access to and use of pre-exposure prophylaxis (PrEP). The PCHP program focuses on increasing diagnosis, treatment, and prevention of HIV at JPAFHC and in the communities served by JPAFHC. The PCHP Prevention Educator will work closely with current JPAFHC patients and staff as well as the communities served by JPA to provide HIV education and health promotion services, outreach, HIV testing on- and off-site, and linkages to care. They will play an integral role on the team to counsel, test, and refer at-risk communities to vital HIV, sexual health, and other primary care services at JPAFHC. They will also work closely with the Outreach Coordinator to provide testing, education, and referrals at outreach and partner events in the community.
