About The Position

The PrEP/PEP Navigator supports individuals at risk for HIV by providing education, HIV testing, care coordination, and navigation services related to PrEP and PEP. This position helps patients access prevention medications, complete required lab work, secure financial assistance when needed, and remain engaged in care through ongoing follow-up and adherence support. Working collaboratively with clinical staff and community partners, the PrEP/PEP Navigator promotes HIV prevention, conducts outreach activities, participates in health education campaigns, and helps ensure patients receive the resources and support needed to successfully maintain their prevention plans.

Requirements

  • Excellent oral and written communication skills.
  • Knowledge of computer applications and great phone etiquette is a must.
  • Ability to multitask, work well in teams, and be highly detailed orientated.
  • Being respectful and culturally competent is also needed, along with being compassionate and identifying with patient needs and concerns.
  • Must have knowledge of PrEP/PEP and ability to educate about PrEP/PEP.
  • Ability to complete PrEP Navigator Training is also a must.
  • Knowledge of HIPAA and be able to communicate knowledge about disease (HIV, HCV, and STDs).
  • Knowledge of social services and referrals for patients.
  • Must be comfortable working in a clinical setting with providers and medical personnel
  • High School Diploma or GED required.
  • At least one year of experience as a Patient Navigator, Case Management, Education or Human Services and great customer service experience is preferred.
  • Must have active Basic 1 Rapid HIV Testing certification from LA County DHSP or open to attending the training.
  • Must have a reliable automobile for use on the job (mileage to be reimbursed).
  • Valid California Driver License and automobile insurance coverage.
  • Subject to a criminal background check prior to employment
  • TB clearance, to be renewed every year.
  • Vaccines and boosters as needed.
  • Compliance with all mandated vaccinations and all boosters is a term and condition of employment.

Nice To Haves

  • Previous experience working in sexual health and HIV education, substance use, and outreach highly desired
  • Previous experience working with LGBTQ+ identified individuals highly desired
  • Applicants fluent in Spanish are strongly encouraged to apply

Responsibilities

  • Provides confidential risk assessment to determine the HIV prevention needs of the clients whether test results are positive or negative; this includes the registration, and planning of prevention services.
  • Provides access to biomedical prevention services (PrEP/PEP), ensuring access, follow-up, and adherence to PrEP/PEP, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance.
  • Provides, and ensures access to the screening of STIs.
  • Collaborates with project staff in advertising and media promotion of the HIV testing program.
  • Responsible for providing Rapid HIV testing on-site and through outreach into the community, where the target population socializes and/or congregate as needed.
  • Provides HIV test results to individuals who have tested for HIV.
  • Performs follow-up risk assessments with individuals who test negative for HIV to reinforce the individuals realistic perception of risk
  • Maintains communication between inter, and intra-agency services providers for successful coordination of services.
  • Maintains records of all activities
  • Attendance at required agency meetings, and external meetings with other local providers.
  • Assists in documenting program activities, and reports outcomes to Department Director along with entering data into Casewatch for LAC DHSP.
  • Community outreach and education
  • Provide first-time medication drop off to client’s within 5-10 mile radius.
  • Perform additional duties as assigned
